Thank you for choosing our office for all your dental needs! We have created a comfortable environment where you can feel confident in the level of dental care you receive. Please read the patient information on this page so that you can be prepared when you arrive for your first visit.

New Patients Forms

We will ask you to come 15 minutes early to fill out paperwork. Alternatively, you can print these forms, fill them out, and bring them to your appointment. If there is any other information you feel we need from you, please tell our staff.

Your First Visit

Your initial exam will last approximately one hour. This visit will help us get acquainted with each other and learn about your goals and desires for your dental care. Generally, digital X-rays will be taken, allowing your dentist to examine the bone structure and tooth position, as well as diagnose any tooth decay, periodontal disease, or other conditions. You will be given recommendations tailored to your specific needs in caring for your teeth and gums. In most cases, we will also clean your teeth at this first visit and provide an evaluation that will outline any existing dental problems and our proposed treatment.

We believe that good dental care begins with open communication. We promise to be honest and straightforward with you while explaining any issues present and while recommending the best treatment options to address those issues. Alternative treatments will also be explained. Working together we can achieve a true partnership, with one common goal – keeping your smile beautiful and healthy!

Financial Policy

Our office is committed to making dental care as accessible as possible for our patients with different financial situations and expectations. Before treatment begins, we will give you a treatment plan that includes a breakdown of all applicable fees. Charges are payable at the time of treatment. If special arrangements are needed, please talk to our office manager who will review our payment options to accommodate your financial needs. Below is a list of the financial options we offer.

Insurance

Our office is committed to helping you maximize your insurance benefits. As a courtesy to our patients, we will submit all insurance claims for services rendered. If requested, we may also submit pre-authorizations for planned treatments to your insurance company. Your estimated patient portion must be paid at the time of service. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee your insurance will provide coverage due to the complexities of insurance contracts. We will allow insurance companies 45 days to render payment, but after 60 days you are responsible for the entire balance, paid in full. If you have any questions, our courteous staff is always available to answer them.

We are a preferred provider with most insurance companies. Please call and ask our staff if you have questions about your insurance.

Fee for Service

No insurance? No problem. We provide a discount for our uninsured patients who pay at the time of treatment. You only pay for the services rendered, instead of paying a monthly fee to insurance companies. We accept the following:

  • Cash
  • Check
  • FSA/HSA benefit card
  • Credit Card (Visa, MasterCard, Discover, American Express)

Financing

We accept CareCredit, a program that allows you to finance your out-of-pocket healthcare expenses for you and your family. If you have questions about applying, please contact our office staff!
